This is intended to be protected for my downline users, how do you confirm they are in your downline? When i tested the “register” link, there is no authentication by myself or my wife, they just default in as a downline member. Effectively, anyone can get at the “protected” content. For now I've just created an additional level so folks can register, be defaulted into the membership level that can't see anything, and then they let us know and we will move them into the “Approved Downline Member” level, but is there a better way to do this? or to require an approval from the site administrator?

You can change the membership so that when people register, they have to be approved first.
WP Membership > Settings
Change the ‘Default Account Status' to ‘Pending'
Now we people register, they will need to be approved by you on the Members tab
